Short-term complex care (2South, 3North and 3South)

In short-term complex care units, we provide low intensity rehabilitation for adults with multiple chronic complex conditions and diagnoses. We work on increasing activity tolerance so patients can return home or be discharged to another level of rehabilitative care in the community.

Specialized wound care is provided to patients with complex wounds including surgical, vascular, pressure injuries and more. We work on wound healing and on improving function so patients can transition back to the community.

Our teams also care for patients who have progressive diseases such as congestive heart failure or chronic kidney disease, and multiple medical conditions that require ongoing monitoring by an inter-professional team. This includes hemodialysis, peritoneal dialysis, tracheostomy, and bariatric care.

Long-term complex care and transitional care (2North, 4North, 4South and 5North)

In long-term complex care units, we care for patients who need specialized services over an extended period to maintain, slow the rate of, or avoid further loss of function. This includes patients with multiple sclerosis, chronic obstructive pulmonary disease, cerebral palsy, tracheostomies and those with neuromuscular disorders requiring ventilator care.

Our teams also provide transitional care to patients so that they can transition back to the community: to a retirement home, a long-term care home, or to their own home with care services.

Palliative care (level 5)

Our palliative care teams work to give life to years, and years to life. They work to improve the quality of life for patients and their loved ones who are facing the complex challenges associated with life-limiting illness.

Palliative care at Bruyère includes end-of-life care, terminal care and ongoing management of pain and symptoms to enhance quality of life for those who are managing life-limiting illness with many years left to live.
