FIPPA Requests
As of January 1, 2012, Bruyère is subject to Ontario's Freedom of Information and
Protection of Privacy Act (FIPPA).
The Act applies to records in the custody and control of Bruyère and does not include personal health information, which is subject to the provincial Personal Health Information Protection Act
(PHIPA).
Informal Process
Bruyère makes information available to the public on a routine basis under public reporting on its website. No formal request under FIPPA is required for information released on a routine basis.
Formal Requests for Information
A request for information in the custody or control of the hospital can be made under the Freedom of Information and Protection of Privacy Act. You may also request access to view or change your personal information. These requests must be made in writing and accompanied by an initial fee of $5. You can submit your request by completing Bruyère’s access request form. Email requests will not be accepted.
