Regional Palliative Consultation Team

The Regional Palliative Consultation Team is a partnership between Bruyère Health and Ontario Health atHome, supporting health care professionals caring for palliative patients in the community. This includes care offered at home, retirement homes, long-term care facilities and community hospitals.

We provide:

  • Consultation in palliative symptom management and complex end-of-life care
  • Advanced palliative care education
  • Coaching and mentoring of health care professionals

When to refer?

Anytime you need help to support patients with a life-limiting disease. Our interprofessional team of nurse practitioners, registered nurse specialists and physicians is here to help 24 hours a day, 7 days a week.


Who can refer?

  • Primary care providers
  • Community hospitals with no/limited palliative care services
  • Long-term care homes
  • Retirement homes and other congregate care settings
  • Home care coordinators and service providers
  • Hospices
