The Regional Palliative Consultation Team is a partnership between Bruyère and Ontario Health atHome that supports family physicians and primary health care teams that care for palliative patients in the community. This includes care offered at home, retirement homes, long-term care facilities and community hospitals
We provide palliative care expertise through:
- Consultation in palliative pain and symptom management and complex end-of-life care
- Advanced Palliative Care Education
- Coaching/Mentoring
When to refer?
Anytime you need help to support patients with a life limiting disease. Our caring and expert team of palliative experts is here to help 24 hours a day, 7 days a week.
Who can refer?
- Primary care providers
- Hospitals with no palliative care services
- Long-Term Care Homes
- Retirement Homes
- Care Coordinators and providers
- Hospices
