Reporting to the Manager, Patient Registration and Flow, this role is accountable for completing the pre-admission, triage and coordination for in-patient admissions in Complex Care, Rehabilitation and Palliative Care.
Establish and maintain effective and efficient patient centered processes to ensure the timely admission of eligible patients in close collaboration with internal and external partners.
Collaborate with team members across programs and sites as well as with other health care facilities across Ontario Health East. Is committed to living the Mission and values of Bruyère.
Is committed to patient and resident safety, on-going learning and continuous quality improvement.
