We are a research team interested in developing decision-making tools to support transfers between long-term care and the hospital.
Do you qualify?
- Are you a long-term care (LTC)resident or caregiver?
- Help us improve the transition from LTC to hospital experience.
- Participate in a two-hour focus group to help us develop a decision-aid tool.
- Participation is voluntary and will not impact your care in any way.
