Transitional care at Greystone
Opened in 2020 as part of the regional COVID-19 response, our Greystone Transitional Care Unit welcomes patients from acute care who need specialized care before returning to the community.
Through transitional care, we provide a bridge for patients to make their way back to the community safely, whether that is returning to their home or transitioning to long-term care.
